Every facility decides what it requires, but the checklist is short and predictable. In general, you'll need an identity and contact profile, the documents your facility asks for, acceptance of that facility's policies, and a clear exclusion-screening result.
The usual requirements
- Identity and contact details
- Documents such as a W-9, certificate of insurance (COI), and immunization records
- Acceptance of each facility's policies
- A clear OIG / SAM exclusion screening result
- A background-check attestation from your company, where a facility requires one
